Admissions formalities

Pre-admissions

For full hospitalisation, to avoid waiting for your admission, please go to the pre-admissions department at 118 Rue Jean Mermoz. To carry out this pre-admission procedure, you will need to bring your administrative papers with you (carte vitale, identity card, health insurance card, etc.).
The pre-admissions office is open on

  • MONDAY: 8.00 AM - 6.00 PM
  • TUESDAY: 8AM - 3PM
  • WEDNESDAY: 8AM - 6PM
  • THURSDAY: 8AM - 5PM
  • FRIDAY: 8AM - 12.30PM

The admission passport

When you undergo surgery, you become a player in your own health care. Your involvement is important. To help you make the most of this new approach to care, the Clinique Juge has created a unique admission passport. It contains a wealth of information to help you make the most of your care, so it's important that you read it carefully. You can download it via the link below:

This booklet will be given to you at your consultation with the surgeon. We ask you to read and sign the associated documents and to bring this passport with you, as it is required for administrative procedures at the clinic (pre-admission and admission).

Reception in the various departments

  • Admissions Ambulatory Surgery Department: Monday to Friday, 7.00 am to 8.00 pm.
  • Digestive endoscopy department: Monday to Friday, 7.45am to 3pm.
  • Fast-tracking department: Monday to Friday, 7.45am to 3pm.
  • Hospitalisation department: Monday to Friday, 7.30am to 5pm.
    A receptionist will welcome you on your arrival at 116, rue Jean Mermoz and direct you to the admissions office, where you will complete the administrative formalities required for hospitalisation .

Administrative documents

On the day of your admission, if the formalities have not already been completed, you must go to the admissions office with the following documents:

  • Your completed and signed pre-admission form (surgery).
  • Photo ID (national identity card, passport, resident's card, etc.).
  • Your up-to-date "carte vitale" or certificate of entitlement.
  • Your mutuelle or supplementary insurance card. Your 100% certificate if you are a beneficiary.
  • A means of payment.

    If you are eligible:
  • Your CMU (Couverture Médicale Universelle) entitlement certificate or your AME (Aide Médicale de l'Etat) card.
  • In the event of an accident at work: the accident at work form issued by your employer.
  • Your free care booklet (for veterans).
  • If you are a national of an EU country: the European Health Insurance Card or form E112.
  • If you are a national of Algeria: form SE 352.
  • If you do not have cover or if you are a national of a country that does not have an agreement with France: deposit cheque and document proving your identity.

Minors and protected adults

The person(s) with parental authority must sign the authorisation to carry out treatment and procedures in the pre-admission/passport booklet. Their presence is compulsory during the stay (decree of 17/01/74). A protected adult may only be hospitalised with the consent of his or her legal representative (articles 488 et seq. of the French Civil Code). When you are admitted, please bring your biological and radiological tests and your current prescription with you. Please note that if you do not complete these formalities, your admission may be postponed or you may be asked to pay a deposit.

Compulsory contributions linked to your stay

In accordance with current regulations, you will be asked to pay certain fees. These are not reimbursed by the Assurance Maladie but may be covered by your mutual insurance company or supplementary health insurance. Ask your health insurance company for details. There are exemptions, particularly for people receiving complementary health insurance or state medical aid. You can find the full list of exemptions on the ameli.fr website.

Fixed hospital charge: €20 per day (including the day of discharge)

The fixed hospital charge represents your financial contribution to the accommodation and maintenance costs incurred by your hospitalisation. It is payable for each day of hospitalisation, including the day of discharge. It does not apply to outpatient stays. The amount of the fixed hospital charge is set by ministerial decree.

Fixed contribution: €24 per stay

Preserving the social security system is everyone's business and, in principle, you contribute to it for each of your healthcare expenses. The part of the cost that remains payable by you is known as the "fixed contribution" or "co-payment".

The fixed contribution of €24 applies to procedures costing €120 or more. You may be asked to pay a co-payment for any procedure under €120.

You may not be billed for any costs other than those corresponding to the care provided or, where applicable, to any special requirements you may have requested. The amount of these special requirements, the list of which is strictly defined by the regulations and includes in particular access to a private room, must be communicated to you before the treatment is carried out.
