The admission passport

When you undergo outpatient surgery, you become a player in your own health care. Your involvement is important. To help you make the most of this new approach to care, the Clinique Juge has created a passport for each department. It contains a wealth of information to help you make the most of your care, so please read it carefully.

This booklet will be given to you at your consultation with the surgeon. We ask you to read and sign the associated documents and to bring this passport with you, as it is required for administrative procedures at the clinic (pre-admission and admission).

Taking care of children

Minors must be accompanied by a legal guardian during consultations and during their stay at the clinic.

For non-routine procedures, such as anaesthesia or surgery, it is compulsory to obtain the consent of both parents (document in the pre-admission file), even in the event of separation.

Law 2002-305 of 4 March 2002 stipulates that parents have joint parental authority(article 372 of the Civil Code), regardless of the couple's legal status.

If the parents refuse, or if it is impossible to obtain their consent, no surgery may be performed (article R. 1112-35, 3rd paragraph of the CSP).
